What is a Family Health Team?
A Family Health Team is a group of health care providers who work together to help keep you and your family healthy and to coordinate the best possible care for your health needs. A team approach offers you better access to appropriate health care in your own community.

What health professionals are part of the TVFHT?
The Thames Valley Family Health Team includes registered nurses, nurse practitioners, dietitians, social workers, and pharmacists. These interdisciplinary health professionals have expertise in specific areas and help support the efforts of your doctor in caring for your health.

How can I see a TVFHT health professional?
You must be a registered patient of one of our participating family health networks or family health organizations, and referred by one of our participating physicians. Your physician will let you know how to book an appointment.

How much will this cost me?
There is no cost to you for any of the services. All the programs and services offered by the Family Health Team are funded by the Ontario Ministry of Health and Long-Term Care.

Will I always see my family physician at my appointments?
Not necessarily. Sometimes you may see another member of our interdisciplinary team.

How do I cancel an appointment?
Please contact the Thames Valley Family Health Team office where you scheduled the appointment.

Why are there medical residents and students at some of the Thames Valley Family Health Team sites?
The UWO Schulich School of Medicine & Dentistry is one of the Thames Valley Family Health Team’s partners. As a team, we believe that our clinics are excellent learning environments for the next generation of medical professionals in our community. There will be times when a medical resident or other healthcare provider will be seeing you at some of our clinics. It is our hope that some of these learners will decide to stay in the Thames Valley after they have graduated.

What do I need to know about Health Cards?
Please bring your health card and give it to the receptionist when you arrive. If you have changed your address or your telephone number, please notify the receptionist. Check the expiry date on your card and arrange for a new one before it expires.
